To schedule a pick-up of your completed sampling kit, please fill in all the required information below. We will schedule the pick-up of your sampling kit via our logistics partner. We will process your request as soon as possible. You will receive an E-Mail from us with the shipping information and any required documentation once the shipment is scheduled.

Please note that same-day pick-up may not be possible depending on the location of pick-up and time of the request.
